Promotional testing is for those that have been approved by their instructor or an authorized trainer to test for the next higher belt rank. If you have been approved to test, you will receive a test form, which you will need to fill out, sign and bring with you to class on your scheduled testing day.

If you have been approved but have not received a test form, you can download and print a blank from below. If you have lost your form, you can get a duplicate form below. Just print, fill out, sign and bring this form with you to class on your scheduled testing day.
